Registration Instructions |
Instructions:
- Complete the online registration you will need to have your driver’s license number. Junior age player will need to have a parent complete this process. The parent will need their driver’s license number.
- Begin registration process by going to the “membership” button on the imvolleyball.org website. From there, decide if you are a new member(never been a member before) or an existing member and click on the appropriate link.
- When you renew or join for the first time with Webpoint/USAVolleyball you will be given the following payment options.
Here are your 2 options:
- If you took advantage of our Online Signature and payment plan via credit card you may immediately print your 2009-2010 membership card at the conclusion of your Online Registration session.
- If you selected "Submit payment with application" please print out your Waiver and Release Form, sign it (For Juniors: both player and parent) and insert check or money order and send to IVA Region Offices: 2595 West Custer Rd Suite A, SLC, UT 84104. Please note that this process may
take 5-7 business days after the time you send your completed forms and required fee IF approved by the Intermountain Region. If payment is not received within
5 days of registering, your membership will be cancelled.
For tryouts:
- Please make copies of your Medical Release Form and Membership card and take them with you to tryouts.

You must be able to provide proof of registration prior to participating in any sanctioned event (including tryouts).
No refunds will be issued. |
| Junior Player |
1. Your parent/guardian will need to complete the online membership process for you.
2. They will need their driver’s license number to complete the process. .
3. Select Payment options (above)
4. If you paid via credit card you can print your membership card immediately. If you selected Submit payment with application you will have to wait 5-7 business days to receive approval and be made a current member in the system. Remember that if the Region does not receive your payment within 5 days of registering, your membership will be cancelled and you will have to register again.
5. Print your medical form and hand in to your Club Director or coach not the Region. |
| Coach, Club Director, Official, Chaperone, Team Representative or an Adult Member affiliated with a Junior Club Team |
1. You will need your driver’s license to complete the process.
2. All must complete and sign online Background Screening form (good for 2 years and part of the online registration). Note: The background screening process can not begin until your signed form and membership fees are paid for.
3. Applicable to all adults affiliated with a junior program: In association with your 2009/2010 season membership renewal, electronic registration now includes the electronic acceptance of the USA Volleyball Junior Club Personnel Code of Ethics, which replaces the coaches code of conduct and is intended for all adults actively participating in junior programming.
Please note that this Personnel Code of Ethics replaces the IVA Region’s Coaches Code of Ethics so you will not have to hand that form into the region. You are still expected to hand in the Participation Agreement form.
4. Medical Release form that you should carry to all events.
5. Participation Agreement (found on website under forms) and hand to club director. You will not be made current until this is filled out and handed in to the Club Director and the Club Director sends the original to the Region offices
If you are a Chaperone
Follow steps above, THEN:
1. IVA Chaperone Responsibility form (found on website under forms) and given to your Club director where the Club Director will send all paper work to the Regional office.
Junior Age Assistant Coaches that are 18 years old or will turn 18 years of age need to do the background screening process. |

1. You will need your driver’s license to complete the process.

| How Electronic Signatures Work Through Our New Online Registration System |
- Be aware that with electronic acceptance it is important that adult members register themselves and that a parent/guardian registers a junior player.
- With the Online signature, the individual completing the registration for a junior player is representing that he/she has legal capacity and authority to act for and on behalf of the minor.
- Coaches and Club Directors should not register their players. It is recommended to Club Directors to have a parent’s meeting in a setting where computers are available to register your junior players online.
- When using the Online Signature to verify who is filling out the form, the Adult or Parent/Guardian will be asked for their Driver’s License Number or the last 4 digits of SS#, so have it on hand before starting the process.
